The Wisdom of Cravings

Whenever I sit with a new patient for an initial intake, I ask about cravings.

From my many conversations about food, appetite and cravings, the most common responses are cravings for salt, or sugar, with many people falling on one end of the preference than the other: “I’m a salt craver” or “I’ve got a sweet tooth”.

However, cravings are so much more than that.

I believe that they are a beautifully intricate process, in which our body is trying to speak to us about what it needs.

Our bodies have developed taste receptors to detect quality nutrients from the environment. While these days sugar is abundant wherever you turn, during our hunter-gatherer times, it was a relatively scarce and highly sought after taste–the taste of ripe fruit, rich with nutrients, the taste of quality calories from carbohydrates, which may have been scarce in times of food shortage or famine.

Salt or “savoury” or umami cravings, often represent a need for more protein. Unfortunately, many of my patients who crave salt (and often calories) find themselves the bottom of a bag of chips, rather than grilling up a chicken breast.

Our modern environment doesn’t necessarily set us up to adequately translate and respond properly to certain cravings. Salted chips were probably not a thing in a natural environment and the only way to satisfy a salt and savoury craving would have been through hunting, consuming meat, or eggs and poultry.

When I was travelling in Colombia I was obsessed with broccoli–it was like I couldn’t get enough of it.

The same thing happened on a month-long trip to Brazil in 2019. Broccoli is rich in vitamin C, sulphur, and certain amino acids. It’s also a decent source of calcium. I’m not sure what nutrient I may have been lacking on my travels, but it’s possible that those cravings meant something for my body. And so I honoured them–I sought out broccoli like it was a magic elixir of health and ate as much of it as I could.

After developing significant iron deficiency after spending a few years as a vegetarian, I became suddenly attracted by the smell of roasting chicken from a local Korean restaurant I was passing by while walking the streets of Toronto.

The wafting smell of roasting poultry was majestic and impossible to ignore. It didn’t smell like sin, or temptation–my body betraying my moral sensibilities or whatever else we often accuse our cravings of—it smelt… like health.

There was no doubt in my mind as the delicious fumes touched my nostrils that I needed to honour my body and start eating meat again. I did and my health and nutrient status has never been better.

Patients will report craving carbs and chocolate the week before their period. The eb and flow of estrogen can affect serotonin levels. A large dose of carbs allows tryptophan, the amino acid that forms the backbone of serotonin, to freely enter the brain. This explains the effect “comfort foods” like starchy warm bread and pasta have on us, creating that warm, after-Thanksgiving dinner glow.

Chocolate is rich in magnesium, a nutrient in which many of us are deficient, that is in higher demand throughout the luteal phase of our cycle, or our premenstrual week.

Cravings are not just nagging, annoying vices, thrust in the path to greater health and iron discipline. They’re complex, intuitive and beautiful. They may be important landmarks on the path to true health and wellness.

Disciplines like Intuitive Eating and Mindful Eating have based themselves on the idea that our bodies hold intuitive wisdom and our tastes, cravings and appetites may be essential for guiding us on a road to health. Through removing restriction and paying more attention to the experience of food, we may be better guided to choose what foods are right for us.

The book The Dorito Effect outlines how our taste cues have been hijacked by Big Food. Like having a sham translator, processed foods stand between essential nutrients and the signals our bodies use to guide us to them. A craving for sweet that might have led you to ripe fruit, now leads you to a bag of nutrient-devoid candy that actually robs you of magnesium, and other nutrients in order to process the chemicals. A craving for salt and umami, or hunger for calories leads you to polish off a bag of chips, which are protein-devoid and laden with inflammatory fats, and only trigger more cravings, and shame.

It’s no wonder that we don’t trust our cravings– we live in a world that exploits them at every turn.

Clara Davis in 1939 was curious about the instintual nature of human cravings and devised a study that was published in the Canadian Medical Assoication Journal (CMAJ). The study was called Self-Selection of Diets by Young Children.

Clara gathered together 15 orphaned infants between 6 to 11 months of age who were weaning from breast-feeding and ready to receive solid food for the first time. These infants, before the study had never tried solid food or supplements. They were studied ongoing for a period of 6 years, with the main study process was conducted over a period of months.

The babies were sat at a table with a selection of simple, whole foods–33 to be exact. The foods contained no added sugars or salt. They were minimally cooked. Not all 33 were presented to each baby at each meal, however the babies were offered an opportunity to try everything.

The foods they were offered were water, sweet milk, sour (lactic) milk, sea salt, apples, bananas, orange juice, pineapple, peaches, tomatoes, beets, carrots, peas, turnips, cauliflower, cabbage, spinach, potato, lettuce, oatmeal, wheat, corn meal, barley, Ry-krisp (a kind of cereal), beef, lamb, bone marrow, bone jelly, chicken, sweetbreads, brains, liver, kidneys, eggs, and fish (haddock).

The nurses who were involved in running the study were instructed to sit in front of the infants with a spoon and wait for them to point at foods that they wanted. The nurses were not to comment on the choices or foods in any way, but wordlessly comply with the infants’ wishes and offer them a spoonful of the chosen foodstuff.

Throughout the study Davis noted that all the infants had hearty appetites and enjoyed eating.

At first, the babies showed no instinct for food choices, selecting things at random, and exploring the various foods presented to them. All of them tried everything at least once (two babies never tried lettuce and one never explored spinach). The most variety of food choices occurred during the first two weeks of the study when they were presumably in their experimentation phase.

Their tastes also changed from time to time, perhaps reflecting some hidden, internal mechanism, growth spurt or nutritional need. Sometimes a child would have orange juice and liver for breakfast (liver is a source of iron, and vitamin C from the orange juice aids in its absorption), and dinner could be something like eggs, bananas, and milk.

Many infants began the study in a state of malnourishment. Four were underweight and five suffered from Rickets a condition caused by extremely low vitamin D. One of the babies with severe Rickets was offered cod liver oil in addition to the other food options. Cod liver oil is a rich source of vitamin D.

The infant selected cod liver oil often for a while, after which his vitamin D, phosphorus and calcium blood levels all returned to normal range, and x-rays showed that his Ricket’s healed.

It is often thought by parents that children, if left to their own devices will eat themselves nutrient-deficient. While that may be true in todays’ landscape of processed frankenfoods, the infants in Davis’ study consumed a diet that was balanced and high in variety. They got 17% of their calories from protein, 35% from fat and 48% from carbohydrates and intake depended on their activity levels.

During the 6 years in which the infants’ eating habits were under observation, they rarely suffered from health issues. They had no digestive issues, like constipation. If they came down with a cold it would last no more than 3 days before they were fully recovered.

In the 6 years, they became ill with a fever only once, an outbreak that affected all of the infants in the orphanage. The researchers noticed their appetites change in response to the illness.

During the initial stages of the fever, they had lower appetites. And, once the fevers began to resolve, their appetites came back with a vengeance. They ate voraciously, and it was interesting that most of them showed an increased preference for raw carrots, beef and beets–which may indicate a need for vitamin A, iron and protein, which are needed for immune system function and recovery.

The habits of the infants to crave and select medicinal foods during times of fever and nutrient deficiency is such compelling evidence of Clara Davis’ craving wisdom hypothesis—were their bodies telling them what they needed to heal?

The self-selected, whole foods diets seems to have a positive impact on the mood and behaviours of the babies, all of whom were living full-time at the orphanage.

A psychiatrist, Dr. Joseph Brennemann wrote an article on them entitled “Psychologic Aspects of Nutrition” in the Journal of Pediatrics on their mood, behaviour and affect, “I saw them on a number of occasions and they were the finest group of specimens from the physical and behaviour standpoint that I have even seen in children of that age.”

In our world we often try to mentalize our food choices: going vegan or low-fat, counting calories, or reducing carbs. We time our eating windows, fast, or try to exert discipline and will over our bodies’ inherent desires.

So often my patients need to be coached through food eliminations, or given meal plans and templates. The art of listening to the body: properly identifying hunger, thirst, fatigue, inflammation, and even emotions like boredom, anxiety, sadness, anger, and hurt, can be a long process.

And yet, I wonder if we clear our palates and offer them a variety of whole, unprocessed, fresh foods, if our bodies will settle into their own grooves–perhaps our health will optimize, our bodies will be able to more readily communicate what they need, our taste receptors and cravings will adjust, and our cravings and appetite will serve the purpose they were meant to–to tell us what we need more of and what need less of or not at all.

I wonder if we listen, what our bodies will tell us.

I wonder if we let them, if our bodies will exhibit the pure instinctual wisdom of nature and the quest for harmony and homeostasis that lies at the heart of our natural world.

“I don’t believe in diseases anymore, I treat stories.

“…No other medical system in the world ever believed in diseases. They all treat everybody as if, you know it’s whether it’s the ancestors or meridians–it’s none of this rheumatoid arthritis, strep throat kind of thing. That’s just this construct that we kind of… made up.”

– Dr. Thomas Cowan, MD

Dr. Cowan is admittedly a (deliciously) controversial figure. His statement, I’m sure, is controversial. But that’s why it intrigues me.

In naturopathic medicine, one of our core philosophies, with which I adhere very strongly, is “treat the person, not the disease”.

And, in the words of Sir William Osler, MD, “It is much more important to know what sort of person has a disease, than to know what sort of disease a person has”.

And, I guess it’s relevant to ask, what is disease in the first place?

I see disease as an non-hard end point, a state that our biological body enters into. On the continuum between perfect health (which may be an abstract and theoretical construct) and death, disease I believe is near the far end of the spectrum.

Disease happens when the body’s proteins, cells, tissues, or organs begin to malfunction in a way that threatens our survival and disrupts our ability to function in the world. For example, a collection of cells grows into a tumour, or the immune system attacks the pancreas and causes type I diabetes.

But, of course there is always more to the story.

What causes disease?

I have heard biological disease boiled down to two main causes: nutrient deficiencies and toxicities. And, I’m not sure how strongly I agree with this, but on a certain level I find this idea important to consider.

However, it is definitely not how Western Medicine views the cause of disease!

Diseases, as they are defined, seem to be biological (as opposed to mental or emotional). They have clinical signs and symptoms, certain blood test results, or imaging findings, and they can be observed looking at cells under a microscope.

Medical textbooks have lists of diseases. Medicine is largely about memorizing the characteristics of these diseases, differentiating one from another, diagnosing them, and prescribing the treatment for them.

As a naturopathic doctor, I see a myriad of patients who don’t have a “disease”, even though they feel awful and are having difficulty functioning. These patients seem to be moving along the disease spectrum, but their doctors are unable to diagnose them with anything concrete–they have not yet crossed the threshold between “feeling off” and “disease”.

Their blood tests are “normal” (supposedly), their imaging (x-rays, MRIs, ultrasounds, etc.) are negative or inconclusive, and their symptoms don’t point to any of the diseases in the medical school textbooks.

And yet they feel terrible.

And now they feel invalidated.

Often they are told, “You haven’t crossed the disease threshold yet, but once you reach the point where you’re feeling terrible and our tests pick it up too, come back and we’ll have a drug for you”.

Obviously not in so many words, but often that is the implication.

Our narrow paradigm of disease fails to account for true health.

Even the World Health Organization states that health is not the mere absence of disease.

So if someone does not have health (according to their own personal definition, values, dreams, goals, and responsibilities), but they don’t have disease, what do they have?

They have a story.

And I don’t mean that what they’re dealing with is psychological or mental or emotional instead, and that their issues are just “all in their head”. Many many times these imbalances are very biological, having a physical location in the body.

Subclinical hypothyroidism, insulin resistance, nutrient deficiencies, chronic HPA axis dysfunction, and intestinal dysbiosis are all examples of this. In these cases we can use physical testing, and physical signs to help us identify these patterns.

An aside: I believe the categories of biological, mental, environmental, and emotional, are false.

Can we have minds without biology? Can we have emotions without minds or physical bodies? How do we even interface with an environment out there if we don’t have a body or self in here?

Aren’t they all connected?

Ok, back to the flow of this piece:

Your story matters.

This is why it takes me 90 minutes to get started with a new patient.

It’s why I recommend symptom and lifestyle habit tracking: so that we can start to pay attention.

It’s why I’m curious and combine ancient philosophies, research (because yes, research is useful, there’s no doubt–we should be testing out our hypotheses), and my own intuition and skills for pattern-recognition, and my matching my felt-sense of what might be going on for a patient with their felt sense of what they feel is going on for them.

Attunement.

I write about stories a lot. And I don’t mean “story” in a woo way, like you talk about your problems and they go away.

No. What I mean is that you are an individual with a unique perspective and a body that is interconnected but also uniquely experienced. And my goal is to get a sense of what it’s like to be you. What your current experience is like. What “feeling like something’s wrong” feels like. What “getting better” feels like.

And all of that information is located within story.

Your body tells us a story too. The story shows up in your emotions, in your physical sensations, in your behaviours (that might be performed automatically or unconsciously), in your thoughts, in your energy, and in the palpation of your body.

No two cases of rheumatoid arthritis are the same. They may have similar presentations in some ways (enough to fit the category in the medical textbooks), but the two cases of rheumatoid arthritis in two separate people differ in more way than they are the same.

And that is important.

We’re so used to 15 minute insurance-covered visits where we’re given a quick diagnosis and a simple solution. We’re conditioned to believe that that’s all there is to health and that the doctors and scientists and researchers know pretty much everything there is to know about the human body and human experience.

And that if we don’t know about something, it means that it doesn’t exist.

When we’re told “nothing is wrong” we are taught to accept it. And perhaps conclude that something is wrong with us instead.

When we’re told that we have something wrong and the solution is in a pill, we are taught to accept that too. And perhaps conclude that something is wrong with our bodies.

But, you know what a story does?

It connects the dots.

It locates a relevant beginning, and weaves together the characters, themes, plot lines, conflicts, heroes, and myths that captivate us and teach us about the world.

A story combines your indigestion, mental health, microbiome, and your childhood trauma.

A story tells me about your shame, your skin inflammation, your anxiety, and your divorce.

Maybe you don’t have a disease, even if you’ve been given a diagnosis.

Maybe you have a story instead.

Alternate Nostril Breathing

Alternate nostril breathing is a yogic breathing practice that can help boost mental clarity and calm the mind.

Hello, everyone. My name is Dr. Talia Marcheggiani and I’m a naturopathic doctor with a special focus in mental health and emotional wellness and today I’d like to show you a special kind of breathing technique for balancing both the right and left hemispheres of the brain.

This breathing technique can help, if we’re feeling overwhelmed and if we’re feeling anxious, to calm the body down, and bring us out of that fight-or-flight stress response. And likewise, if we’re feeling really depleted and tired it can bring up our energy levels and give us a boost of clarity and concentration.

In Ayurvedic medicine—this is the ancient Indian medicine—it’s believed that each nostril supplies oxygen for each side of the brain: the right and left hemispheres. And we know from neuroscience that the right hemisphere of the brain is more involved in creativity and relationships and intuition, whereas the left hemisphere is more analytical and language-based and reason-based. The right side of the body is more masculine and Yang in nature and the left side is more Yin in nature. It’s believed in Ayurvedic medicine that each nostril controls oxygen to each side of the brain.

So this only takes about 3-5 minutes and I encourage you to try it just as a meditation technique if you’re beginning a meditation practice, or to calm the body or something to do before sleep where you can balance the brain and relax the body before drifting off into a sounder and better night’s sleep.

So if you take a seat on the floor or on a surface like your bed, you can sit in a cross-legged position. Or if you’re sitting in a chair have your feet flat on the floor and your back straight. So just have your shoulders back and down and keep your back in a straight, upright position. It shouldn’t feel too stiff. You should have a natural curve at the back of your spine.

Have your left arm either palm facing down or palm facing up on your left knee.

Take your right hand. You’re going to take the index fingers, the peace sign, and you’re going to place them right between your eyebrows. This is the “Third Eye”. You’re going to have your ring finger and your thumb free.

Just take a moment. You can close your eyes or you can watch what I’m doing here, but I’ll talk you through it. Now I just want you to feel your breath deepen. So send your breath to the lower abdomen. If you want you can place your hand on the lower abdomen and just feel the breath move the abdomen in and out as you breathe.

Deeper breaths have a calming and more calming effect on the body. They also stimulate the vagus nerve, which is the nerve that runs from our brain to our digestive system and is involved in calming the body down and bringing it into that rest and digest state.

When you’re ready take one deep, cleansing breath in and exhale and then block off the right nostril with your thumb. Breathe in through the left nostril. Using the ring finger close both nostrils and then release your thumb and the right nostril and exhale.

Breathe in through the right nostril… block it off…hold… and release and breathe out through the left. Now try it again. So, breathe in through the left, block it off, and exhale through the right. Inhale through the right. Block it off. Exhale through the left. Inhale again through the left. Block it off. And exhale through the right. So, if you want you can stop here and just continue on your own, but let’s continue together for another two minutes. I’ll just guide you through it. Inhale through the right nostril. Block. Exhale through the left. Inhale. Block. Exhale through the right. Inhale. Block it off. Exhale through the left. Inhale through the left. Block. And exhale through the right. Inhale. Block. And exhale.

One more cycle. Inhale. Block. Exhale. Inhale. Block. And exhale. Now place both hands on your knees, facing up or down. Keep your eyes closed and take a couple more inhales, breathing into the belly. Notice how you’re feeling; if you’re feeling a sense of calm, or an increased sense of clarity. You probably noticed that one of your nostrils was more blocked than the other one and that through doing this practice you managed to either switch which nostril was activated or clear both for better breathing.

Shinrin-Yoku: The Art of Forest Bathing

In this video I talk about the Japanese art of Shinrin-Yoku, or “forest bathing” and how incorporating this practice into your routine can set you up for improved mental health and stress management.

Hello, everybody, my name is Dr. Talia Marcheggiani and I’m recording to you guys from the Humber River in Toronto. It’s December 24th, so it’s Christmas Eve Day and I’m just walking my dog, Coco, and I just wanted to make a quick video to talk to you guys about the benefits of something called Forest Bathing.

So, Forest Bathing is a concept that originated in Japan. In Japanese it’s called, and I’m going to butcher this pronunciation: “Shinrin-yoku”. And this is a practice to relieve stress, which is taken on by the Japanese and is also something that’s gaining more recognition in Western society as well.

One of the theories of the prevalence of chronic stress and symptoms like adrenal fatigue and the hormone dysregulation that comes with that and the mental health issues that come with that is our disconnection from nature. And, so, taking some time—and I recognize sometimes just starting with an hour a week, but optimally three hours a week, of time where you’re just slowly exploring nature and walking in a natural setting such as this. And this is just near Old Mill station in Toronto, and there’s many places like this if you look at Discovery Walks Toronto online you could find tons of really cool places to explore that are a TTC ride away.

And, so, being in a natural setting decreases the stress hormone cortisol as well as increases our ability to get some physical activity. The reason I’m recording it now in December is because a lot of my patients will tell me that they’re really active in the summer months, but when it comes to winter, or when winter months set in, they tend to stay indoors. And the temperature regulation that comes with getting outside, put on a toque, put on a park and some water-proof boots—these are not the best—put on some water-proof boots, a toque, parka, some mittens, and get outside. It helps our body deal with the cold, the decrease in temperature and it can increase our thyroid and our immune system. So, actually getting outside, even when you don’t really want and your intuition is telling you to stay indoors, is actually a good idea for your immune system.

And the stress that comes with the holiday season and this time of year, getting outside becomes more important than ever. So being in a natural setting, in the trees, increases the amount of negative ions that you’re surrounded by, and so when we’re surrounded by technology or are in indoors, stressful environments, there are more positive ions.

Being outside increases your serotonin and oxytocin levels. So, those are your feel-good hormones and it decreases your cortisol. So I just wanted to say hi to everybody and to maybe inspire some of you go for a walk, either today or Christmas Day, and get some Shrinrin-yoku, some much-needed forest bathing.

Having a dog obviously helps. “Coco, Coco, Hey! Look!” Having a dog obviously helps because you’re forced to get outside but even just taking yourself, your kids, your family out, is a great thing to do. Get some snowshoes on, some cross-country skiis, or just some water-proof boots and head out onto the trails.

Is Wu Wei the Modern-Day Stress Solution?

New Doc 61_1When a successful person is asked to share the secrets of his or her success, the unanimous response is that success requires “effort” and “hard work”. Other popular euphemisms are that success demands “sweat, blood and tears” or “1% inspiration, 99% perspiration”. In grade school we’re lectured on working harder and reprimanded for not trying hard enough. Put in the time and effort to get the grades that will land you the job that will require similar or increasing levels of time and effort. Olympic athletes are lauded as modern-day heroes for their early mornings, punishing workouts and restricted diets. Our society reveres those who put in 80-hour work weeks, despite their implausibility, as we all need to sleep, eat and defecate, which take up hours that are obviously not spent working. It’s a shame to some that we’re limited by these earthly, time-consuming bodies that have been the disdain of capitalism since it’s inception. I see more youth in my practice, who are already bearing the weight of society’s expectations. They are working hard, as instructed, and this “hard work” is taking a physical, mental and emotional toll on their lives. But what can they do? Success requires effort, goes the mantra of our times.

This effort shows up on faces and in bodies. Our minds exhaust us with chastising inner talk; teeth clench, jaws grind and shoulders hunch around ears, creating a new neck-less species, The Hard Worker, inhabiting the earth in increasing numbers.

70% of people today admit to being under stress. When I asked my patients if they feel “stressed”, many shrug, deny or tell me their stress is nothing that they can’t handle, and then proceed to rank their stress levels as 6 out of 10 or higher, 10 being a hair-pulling, crazy-eyed, stressed-out-of-their-wits state—I find it significant that being more than halfway to our limits still qualifies us as not being stressed.

It’s likely that admitting to stress and overwhelm is a weakness in our effort-driven society. If the bodies we inhabit fail to perform, we’ll be replaced by someone more able-bodied, or someone more able or willing to push their limits and sacrifice their health for short-term success—or maybe a machine. Machines don’t need paid vacation or sick leave. While the future seems bleak, it certainly helps with the healing business, as 75-90% of doctor’s visits are either directly or indirectly stress-related. We know that stress has the power to wreck havoc on the body, contributing to diseases, inflammation, decreased immunity, proliferation of cancer and premature aging. We know that stress is implicated in the rising incidence of mental health conditions. We know that things can’t go on as they are, the system is simply not working.

And so when modern society is failing us, it helps to turn to ancient ideas, before it all went wrong, such as Taoist philosophy, to look for answers. The Taoist principles of wu wei, or “effortless action”, tell us that action does not always originate from effort and stress. Effective actions, like creativity and good ideas, can occur spontaneously and of their own accord.

In a society where relentless growth and production are imperative for the survival of the economy, it’s hard to image any action in the absence of sheer effort. Incessant production can’t rely on the eb and flow cycles of natural inspiration and creativity to dictate when, how much or how hard we work. And yet, this perhaps says more about the societal necessities of the work available to us, and our enjoyment (or lack thereof) of such work, than it does about human inspiration. I’ve been planning this blogpost for a while and although it might have been written sooner if set to a deadline, I’ve eventually gotten around to it; here it is. It is a fact that when things need to get done, they will. The doing might happen later than we’d like it to, and yet it still happens, inspired by a genuine desire or necessity, rather than pressure-cooker of stress.

I’m often asked, as a doctor, to help support my patients’ bodies in periods of intense stress—”periods” that have gone on for years with no apparent end in sight. While there are several remedies that can help the body recuperate from the wear and tear of effort or help the adrenal glands secrete more cortisol to continue producing more and faster, there’s also only so much that can be squeezed out of tired organs. Oftentimes, as I’ve written before, the path to healing is paved with introspection and a serious reconsideration of lifestyle. Can we continue to produce and strive at our current rates and still expect to feel fit, healthy and energized? We can build faster computers and smarter phones but our bodies are very much limited to the tools nature has slowly evolved over time, including the natural medicines available to us. Perhaps our lifestyles, like the economy and the stress on the environment, are simply unsustainable. Perhaps it’s time to question how many of the activities in our lives are worth the effort.

While it may not be possible to quit our jobs, pack up and move to the Bahamas, perhaps there are small nooks in our routines where wu wei might fit and flourish. It may be possible to ease up on our own expectations of ourselves, or give up some of our conventional ideas of success. After all, is the journey to success worth slogging if we won’t be happy or healthy when we get there? Finding space in our lives to allow action to arise spontaneously may be crucial in doing the necessary, healing work of stress-management.

Applying wu wei might mean examining the intentions behind our actions and our current lifestyles. Here are some questions to ask yourself.

  1. When is effort appropriate and when is it wasted?

The Dangerous Single Story of the Standard Medical Model

IMG_6021A singular narrative is told and retold regarding medicine in the west. The story goes roughly like this: the brightest students are accepted into medical schools where they learn­—mainly through memorization—anatomy, physiology, pathology, diagnostics, microbiology, and the other “ologies” to do with the human physique. They then become doctors. These doctors then choose a specialty, often associated with a specific organ system (dermatology) or group of people (pediatrics), who they will concentrate their knowledge on. The majority of the study that these doctors undergo concerns itself with establishing a diagnosis, i.e.: producing a label, for the patient’s condition. Once a diagnosis has been established, selecting a treatment becomes standardized, outlined often in a cookbook-like approach through guidelines that have been established by fellow doctors and pharmaceutical research.

The treatment that conventional doctors prescribe has its own single story line involving substances, “drugs”, that powerfully over-ride the natural physiology of the body. These substances alter the body’s processes to make them “behave” in acceptable ways: is the body sending pain signals? Shut them down. Acid from the stomach creeping into the esophagus? Turn off the acid. The effectiveness of such drugs are tested against identified variables, such as placebo, to establish a cause and effect relationship between the drug and the result it produces in people. Oftentimes the drug doesn’t work and then a new one must be tried. Sometimes several drugs are tried at once. Some people get better. Some do not. When the list is exhausted, or a diagnosis cannot be established, people are chucked from the system. This is often where the story ends. Oftentimes the ending is not a happy one.

On July 1st, naturopathic doctors moved under the Regulated Health Professionals Act in the province of Ontario. We received the right to put “doctor” on our websites and to order labs without a physician signing off on them. However, we lost the right to inject, prescribe vitamin D over 1000 IU and other mainstay therapies we’d been trained in and been practicing safely for years, without submitting to a prescribing exam by the Canadian Pharmacists Association. Naturopathic doctors could not sit at the table with the other regulated health professions in the province until we proved we could reproduce the dominant story of western medicine—this test would ensure we had.

Never mind that this dominant story wasn’t a story about our lives or the medicine we practice—nowhere in the pages of the texts we were to read was the word “heal” mentioned. Nowhere in those pages was there an acknowledgement about the philosophy of our own medicine, a respect towards the body’s own self-healing mechanisms and the role nature has to play in facilitating that healing process. It was irrelevant that the vast majority of this story left out our years of clinical experience. The fact that we already knew a large part of the dominant story, as do the majority of the public, was set aside as well. We were to take a prescribing course and learn how primary care doctors (general practitioners, family doctors and pediatricians), prescribe drugs. We were to read accounts of the “ineffectiveness” of our own therapies in the pages of this narrative. This would heavy-handedly dismiss the experience of the millions of people around the world who turn to alternative medicine every year and experience success.

We were assured that there were no direct biases or conflict of interests (no one was directly being paid by the companies who manufacture these drugs). However, we forget that to have one story is to be inherently and dangerously biased. Whatever the dominant story is, it strongly implies that there is one “truth” that it is known and that it is possessed by the people who tell and retell it. Other stories are silenced. (Author Chimamanda Ngozi Adichie describes this phenomenon in her compelling TED Talk, “The Danger of a Single Story”).

Despite the time and money it cost me, taking the prescribing course afforded me an opportunity to step outside of the discouraging, dominant story of the standard medical model and thicken the subordinate stories that permeate the natural and alternative healing modalities. These stories began thousands of years ago, in India and in China, at the very root of medicine itself. They have formed native ancestral traditions and kept entire populations and societies alive and thriving for millennia. Because our stories are not being told as often, or told in the context of “second options” or “last resorts”, when the dominant narratives seem to fail us, the people who tell them run the risk of being marginalized or labeled “pseudoscientific.” These dismissals, however, tell us less about The Truth and more about the rigid simplicity of the singular story of the medical model.

It is frightening to fathom that our body, a product of nature itself, encompasses mysteries that are possibly beyond the realm of our capacity for understanding. It’s horrifying to stand in a place of acknowledgement of our own lack of power against nature, at the inevitability of our own mortality. However, if we refuse to acknowledge these truths, we close ourselves off to entire systems that can teach us to truly heal ourselves, to work with the body’s wisdom and to embrace the forces of nature that surround us. The stories that follow are not capital T truths, however, they can enrich the singular story that we in the west have perpetuated for so long surrounding healing.

The body cannot be separated into systems. Rather than separating depression and diarrhea into psychiatry and gastroenterology, respectively, natural medicine acknowledges the interconnectivity between the body’s systems, none of which exist in a vacuum. When one system is artificially manipulated, others are affected. Likewise, an illness in one system may result in symptoms in another. There have been years of documentation about the gut-brain connection, which the medical model has largely ignored when it comes to treatment. The body’s processes are intricately woven together; tug on one loose thread and the rest either tightens or unravels.

We, as products of nature, may never achieve dominion over it. Pharmaceutical drugs powerfully alter the body’s natural physiology, often overriding it. Since these drugs are largely manmade, isolated from whole plants or synthesized in a lab, they are not compounds found naturally. Despite massive advances in science, there are oceans of what we don’t know. Many of these things fit into the realm of “we don’t know what we don’t know”—we lack the knowledge sufficient to even ask the right questions. Perhaps we are too complex to ever truly understand how we are made. Ian Stewart once wrote, “If our brains were simple enough for us to understand them, then we’d be so simple that we couldn’t.” And yet, accepting this fact, we synthesize chemicals that alter single neurotransmitters, disrupting our brain chemistry, based on our assumption that some people are born in need of “correcting” and we have knowledge of how to go about this corrective process. Such is the arrogance of the medical model.

There are always more than two variables in stories of disease and yet the best studies, the studies that dictate our knowledge, are done with two variables: the drug and its measured outcome. Does acetaminophen decrease pain in patients with arthritis when compared to placebo? A criticism of studies involving natural medicine is that there are too many variables—more than one substance is prescribed, the therapeutic relationship and lifestyle changes exert other effects, a population of patients who value their health are different than those who do not, the clinical experience is more attentive, and so on. With so many things going on, how can we ever know what is producing the effect? However, medicine is limited in effect if we restrict ourselves to the prescription of just one thing. This true in herbalism, where synergy in whole plants offers a greater effect than the sum of their isolated parts. By isolating a single compound from a plant, science shows us that we may miss out on powerful healing effects. Like us, plants have evolved to survive and thrive in nature; their DNA contains wisdom of its own. Stripping the plant down to one chemical is like diluting all of humanity down to a kidney. There is a complexity to nature that we may never understand with our single-minded blinders on.

Studies are conducted over the periods of weeks and, rarely, months, but very rarely are studies done over years or lifetimes. Therefore, we often look for fast results more than signs of healing. This is unfortunate because, just as it takes time to get sick, it takes time to heal. I repeat the previous sentence like a mantra so patients who have been indoctrinated into a medical system that produces rapid results can reset expectations about how soon they will see changes. Sometimes a Band-Aid is an acceptable therapy; few of us can take long, hard looks at our lives and begin an often painful journey in uncovering what hidden thought process or lifestyle choices may be contributing to the symptoms we’re experiencing. However, the option of real healing should be offered to those who are ready and willing.

When we study large masses of people, we forget about individuality. When we start at the grassroots level working with patients on the individual level, we familiarize ourselves with their stories, what healing means to them. In science, large studies are favoured over small ones. However, in studies of thousands of people, singular voices and experiences are drowned out. We lose the eccentric individualities of each person, their genetic variability, their personalities, their preferences and their past experiences. We realize that not everyone fits into a diagnostic category and yet still suffers. We realize that not everyone gets better with the standard treatments and the standard dosages. Starting at the level of the individual enables a clinician to search for methods and treatments and protocols that benefit each patient, rather than fitting individuals into a top-down approach that leaves many people left out of the system to suffer in silence.

It is important to ask the question, “why is this happening?” The root cause of disease, which naturopathic medicine claims to treat is not always evident and sometimes not always treatable. However, the willingness to ask the question and manipulate the circumstances that led to illness in the first place is the first step to true and lasting healing; everything else is merely a band-aid solution, potentially weakening the body’s vitality over time. No drug or medical intervention is a worthy substitute for clean air, fresh abundant water, nutritious food, fulfilling work and social relationships, a connection to a higher purpose, power or philosophy and, of course, good old regular movement. The framework for good health must be established before anything else can hope to have an effect.

The system of naturopathic medicine parallels in many ways the system of conventional pharmaceutical-based medicine. We both value science, we both strive to understand what we can about the body and we value knowledge unpolluted by confusing variables or half-truths. However, there are stark differences in the healing philosophies that can’t be compared. These differences strengthen us and provide patients with choice, rather than threatening the establishment. The time spent with patients, the principles of aiming for healing the root cause and working with individuals, rather than large groups, offer a complement to a system that often leaves people out.

There are as many stories of healing and medicine as there are patients. Anyone who has ever consulted a healthcare practitioner, taken a medicine or soothed a cold with lemon and honey, has experienced some kind of healing and has begun to form a narrative about their experience. Anyone with a body has an experience of illness, healing or having been healed. Those of us who practice medicine have our own experience about what works, what heals and what science and tradition can offer us in the practice of our work. Medicine contains in its vessel millions of stories: stories of doubt, hopelessness, healing, practitioner burnout, cruises paid for my pharmaceutical companies, scientific studies, bias, miracle cures, promise, hope and, most of all, a desire to enrich knowledge and uncover truth. Through collecting these stories and honouring each one of them as little truth droplets in the greater ocean of understanding, we will be able to deepen our appreciation for the mystery of the bodies we inhabit, learn how to thrive within them and understand how to help those who suffer inside of them, preferably not in silence.

Finding the time and energy to cook during the winter can be tough. However, one of the best things you can do for your health (and wallet) is cook meals at home. Knowing a few slow cooker recipes is essential for getting through a cold, dry winter. Soups, stews and curries are warming and easy to digest; these healthy comfort foods help us feel energized and keep our immune systems strong. Slow cooking is a great way to prepare healthy, hearty meals while you watch the Mindy Project or spend the day working or hiking in the forest. This recipe makes a good amount of eggplant masala, which is perfect for leftovers and lunches. The meal is gluten free (if served with rice) and dairy free as well as vegan. It is high in healthy fibres, healthy fats (if cooked in coconut oil or avocado oil) and protein. The recipe is an adaptation from one I learned at a cooking class I took while traveling in Northern India.

Eggplant Masala (from North India)

Ingredients:

4 medium-sized eggplants

4 large tomatoes (or a can of diced tomatoes)

3 white onions

10 cloves of garlic

1 can of chickpeas or lentils

5 tbs mustard oil, coconut oil or another heat-stable oil (I had avocado oil on hand)

4 tsp ground coriander

3 tsp ground turmeric

2 tsp red chilli peppers

1 tsp salt

1 tsp garam masala

1 tsp mustard seeds

1 tsp cumin seeds

1 tsp fenugreek seed (I couldn’t find fenugreek when I was in the grocery store, and my dog was waiting, tied outside, so I didn’t include it in this recipe. The recipe still tasted lush without it).

a blender, stove and slow-cooker (optional)

Directions:

Heat oil on medium heat, add in the spices to release the flavours.

Blend together tomatoes, onions and garlic. Chop eggplants into 1 inch squares.

Add all the ingredients – eggplants, oil + spices, and vegetable blend, including the lentils/chickpeas – to a slow-cooker and cook on low for 6 hours or on high for 4. Take a walk with your family, meet your friend for coffee, ride your bike, have a bubble bath or leave for work. Another option is cooking the ingredients on a covered pan over medium-high heat for 40 minutes to 1 hour, or until eggplants are soft.

When ready, serve over rice or with chapati or naan.

In Traditional Chinese Medicine, the Spleen organ can be more closely equated with the pancreas, rather than the western spleen, whose main function is simply to store blood. The TCM Spleen is responsible for digesting food and converting it into energy, much in the way the pancreas releases insulin to allow the body’s cells to absorb ingested glucose, providing the cells with energy. Unlike the pancreas, however, the TCM Spleen is also responsible for providing warmth and vitality to the body, providing energy for immune function and the mental energy to produce industrious and creativity work. The Spleen belongs to the Earth Element and its main season is late Summer and early Fall, right around the time that students dust off their backpacks and head back to school.

Acupuncture FAQ

Naturopathic doctors employ a variety of healing modalities or, as we like to say, “tools in our toolbox”, to help the body heal itself. One of my favourite healing modalities is acupuncture. While I used to cringe at the thought of spending another 2 hours being poked and prodded by my classmates in acupuncture class I’ve since overcome my fear of needles and have come to embrace the amazing powers of acupuncture to gently restore balance and healing to the body. Under Acupressure

One of my favourite courses, since I began my studies to become a naturopathic doctor, has been Asian Medicine. I am fascinated by the fact that this holistic form of medicine treated billions of people for the past 3000 years, how it remains a viable form of medicine and how, only now, when we put it to the test of our Western research methods, we find that it does work, but we’re still not sure how.
